(a) Jobsite - On or about May 10, 2016 employees exposed to being struck - by flying wood chips when operating a Vermeer HG4000 horizontal grinder. - Warning zones were not marked in areas where there is a potential for thrown objects. - Audible alarm was inoperative which warns employees that machine is starting and to stay away from thrown object zones. |

Citation text line | 29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative. (a) Vermeer HG 4000 Wood Grinder - On or about May 10, 2016 The employer did not establish an energy control program containing a specific energy control procedure for locking and tagging out the Vermeer HG 4000 grinder during maintenance and/or servicing. |
