(a) In front of the gas station - On or about September 3, 2015 two employees were exposed to a fall hazard while repairing and replacing a lamp and ballast while working inside a Chevrolet C6500 Bucket Truck. These employees were elevated 19 feet 5 inches above concrete without fall protection. |
