(a) Rightside Warehouse- On or about January 16, 2013 wood boards and wood boards with nails fell from the ceiling and stuck an employee. On or about January 22, 2013 wood boards and wood boards with nails were observed on the work floor creating a fall hazard for employees. |

This main circuit breaker boxes labels were washed off because of exposure to rain water form leaks in the building roof. |
