(a) Below a large bill board - On or about June 15, 2016 employees were exposed head injuries while other employees were working on scaffolds above them handling cinder blocks. None of the employees on-site were wearing hardhats. |

29 CFR 1926.451(c)(1)(ii): Guys, ties, and braces shall be installed according to the scaffold manufacturer's recommendations or at the closest horizontal member to the 4:1 height and be repeated vertically at locations of horizontal members every 20 feet (6.1 m) or less thereafter for scaffolds 3 feet (0.91 m) wide or less, and every 26 feet (7.9 m) or less thereafter for scaffolds greater than 3 feet (0.91 m) wide. The top guy, tie or brace of completed scaffolds shall be placed no further than the 4:1 height from the top. Such guys, ties and braces shall be installed at each end of the scaffold and at horizontal intervals not to exceed 30 feet (9.1 m) (measured from one end [not both] towards the other). (a) Below a large bill board - On or about June 15, 2016 employees were exposed to a fall hazard due to the fact a scaffold 26 feet above the ground was not tied to the structure. This scaffold was 4 tiers high and not tied to the structure. |

29 CFR 1926.454(a): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include the following areas, as applicable: (a) Washington Masonry - On or about June 15, 2016 employees were exposed to a fall hazard while working on a scaffold without training on how to recognize the hazards associated with this scaffold. This scaffold: 1) had no base-plates installed 2) had no ladders installed 3) employees were not wearing hardhats while other employees laid blocks above them 4) had no ties to the structure at 20 feet (4 tiers) high. |
