Citation text line 29 CFR 1926.453(b)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift. Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e). (a) Holiday Inn Express & Suites - On or about October 29, 2014 an employee was exposed to a fall hazard while working in a JLG-460SJ Telescopic Boom Lift serial no. 627268. This employee was elevated 37 feet 2 inches above concrete without fall protection.

Citation text line 29 CFR 1926.454(a): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include the following areas, as applicable: (a) R & R Steam Cleaning - On or about October 29, 2014 an employee was exposed to potential fall hazard while working in a JLG-460SJ Telescopic Boom Lift serial no. 627268. One employee stated he had never received any training about the hazards of associated with working on an aerial lift such as: electrocution, tip-over, high winds, falls, collisions, explosions, fires, and damage machinery.
