Citation text line 29 CFR 1910.95(g)(1): The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels: (a) Tank Shop - On or about June 26, 2014, a welder was exposed to continuous noise at 166.2% of the permissible daily noise (eight hour time weighted average sound level of 90 decibels) or an equivalent sound level of approximately 94 decibels during the 347 minutes sampling period on 6/26/14. Exposure calculations included a zero increment for the time not sampled. (b) Tank Shop - On or about June 26, 2014, a welder was exposed to continuous noise at 128.2% of the permissible daily noise (eight hour time weighted average sound level of 90 decibels) or an equivalent sound level of approximately 92 decibels during the 357 minutes sampling period on 6/26/14. Exposure calculations included a zero increment for the time not sampled. (c) Tank Shop - On or about June 26, 2014, Welder was exposed to continuous noise at 194.5% of the permissible daily noise (eight hour time weighted average sound level of 90 decibels) or an equivalent sound level of approximately 95 decibels during the 357 minutes sampling period on 6/26/14. Exposure calculations included a zero increment for the time not sampled. (d) Tank Shop - On or about June 26, 2014, a welder was exposed to continuous noise at 74.8% of the permissible daily noise (eight hour time weighted average sound level of 90 decibels) or an equivalent sound level of approximately 88 decibels during the 384 minutes sampling period on 6/26/14.
