Employee was assisting in cleaning up an area of old parts, pipes, etc. As he was carrying a piece of iron on his shoulder, he felt something on his face & neck and brushed it aside with his gloved hand. Later, after arriving home, he noticed 3 bite marks on his neck & face. He reported the incident the next day at work and went to the company doctor at that time.

Employee was cutting handrails with abrasive cutoff wheel. Handrail pipe flexed and pinched the cutoff wheel causing it to shatter and debris struck the employee in the lower right forearm. The forearm was lacerated by the debris.

A coal mill explosion occurred within our coal mill. No injuries of plant personnel. Contributing factors were a diff. of about 20F between control room thermocouple and manual readings. Operational set point for coal mill exit temp. is about 180F and with this diff. there was the possibility that the temp reached 200F.

EE was making his routine rounds around the kiln when he felt something in his left eye. EE was wearing safety glasses so the object must have been blown in by the wind. A small speck was seen on his eye so, being it was night shift, he was sent to the hospital to have particle removed. The attending physician flushed out the eye and suggested he get some occular fluid.

EE was hitting the neck of the finish mill with a sledge hammer (because of a neck choke up) trying to unstop the neck. Cement falling from above got into his left eye. He was wearing safety glases at the time.

Mechanic was using a 1 impact wrench with a 2 3/8 socket. The safety pin that held socket on was a welding rod, which was hanging out of socket. The mechanic's hand (which was in glove) was slashed and cut by the rod while socket was in motion. 12 stitches were required to seal wound on right hand.

Key Temp employee contractor was en route to the cement silos to change silos. When exiting the clinker silos on the north end, he slipped on granular clinker. This caused him to lose footing and apparently twist ankle. Two days later by x-ray a doctor found a broken bone in foot.
