Complaint What Happened I was billed incorrectly by the hospital. I was told by the hospital that since I was admitted into the hospital for life threating condition that my insurance company would be bill as " in network ''. The hospital coded the bill stating that it was " out of network '', leaving he a large unpaid balance. After fighting with them for over 6 months by insurance agreed to pay the balance and informed me that the bill was taken care of. I have not heard anything nor received any bills from the hospital since. I was notified by XXXX that I had been placed into collections by the hospital for {$120.00} for the XXXX co-pay. My insurance company says that since I was admitted the XXXX co-pay should have been waved. I just spoke to the hospital and was told again, that since I was " out of network '' the co-pay was not waived, but could not explain why ( nor did they deny ) that they never sent me a bill for the {$120.00}. Being placed into collections caused my credit score to drop from XXXX to XXXX. This drop is causing me to withdraw an application at a local bank for a second mortgage on my house because of the negative effect the drop in score is causing to the interest rate the bank can offer. I just paid the {$120.00} to the collection agency ( that is owned by the hospital ), this is a debt that I am certain that I did not owe but restoring my credit score is far more valuable to me than {$120.00}. The inability of the hospital to code their bills correctly has cause not only my credit score to drop by XXXX points but can have long term adverse effect by showing that at one point I was sent to a collection agency for failure to pay a bill. I asked the person at the collection agency if she could note that the collection was entered in error, she replied that she could not, only that it had be satisfied. No one should have the power to damage anyone 's credit history due to their own lack of internal controls and not have the power to remove the credit bureau entry.

Complaint What Happened I have contacted Medical Financial Services Incorporated and the original debtor, XXXX XXXX XXXX, about several accounts that have been placed for collection and currently appear on my credit report. Because over {$7000.00} in payments from my insurance company were either incorrectly applied or never applied at all to my account until after I called them and because I never received bills for these hospital encounters, these accounts went into collections and never should have. Since I have begun disputing these bills, over {$6000.00} in collections have been removed or updated from my account ; however collections for two accounts remain. The first is bill for {$180.00} that had been affecting my credit for 2 years but I only became aware of recently when I tried to apply for a loan. I contacted the collection agency and disputed the validity of the debt after contacting my insurance company. To make a long story short, my insurance payment was not fully applied to this debt and I actually did n't owe anything. This account had already been sent to collections and reported to the credit bureaus before XXXX even filed the insurance claim. The debt was promptly removed, but again, it had been on my credit report for 2 years at that time. Another one is for {$630.00} for an emergency room visit on XXXX/XXXX/XXXX. I did n't get a bill for this encounter and assumed that my insurance provider had paid it off until it appeared on my credit report in XXXX XXXX. When I asked the hospital for a detailed bill because of the previous error that had been made on my account, I noticed that I had been charged for the same test twice, in the amount of {$230.00} each time. I sent a message through my online account at XXXX and never received a response. That debt remains on my credit report despite the fact that I believe there is a billing error on my account. The final bill is for an emergency XXXX that I had on XXXX XXXX, XXXX. An entry was made on my credit report for {$6500.00} on XXXX, XXXX. Again after contacting my insurance provider, I learned that they had sent XXXX several payments and my responsibility was actually only {$910.00}. The payments were not applied to my account until after I contacted XXXX XXXX XXXX and Medical Financial Services Incorporated later in XXXX XXXX. I never received a bill and I assume because the payments were not posted in a timely manner, this last account actually shows it is included in my current balance at XXXX. I am already in a payment plan with them for charges from another recent XXXX. The {$910.00} bill is included on my account as an amount that has been billed in the last 0-30 days, therefore, according to XXXX 's own Financial Assistance Policy and because they are a 501 ( c ) 3 religious affiliated organization, no patient accounts will be sent to collections until they are more than 121 days past the post discharge billing date ( see page 3 of the Financial Assistance Policy that I have attached. )

Complaint What Happened My name is XXXX XXXX, a federally protected consumer, and I am making this complaint against Medical Financial Services , Inc for committing identity theft. I have never given Medical Financial Services any permission to use any of my identifying information to commit mail fraud by contacting me about an alleged debt they claim I owe. I am aware that the that in accordance with Article 9, the assignment of a defaulted note is illegal. Both the assignee and the assigned are both liable for violations under 15 U.S. Code 1692e ( 12 ). I have never received any documentation requesting validation from Medical Financial Services before they committed multiple violations under the FCRA and the FDCPA by furnishing this alleged debt to my consumer report including but not limited to 15 U.S. Code 1692d ( 4 ) which the FTC defines as debt parking. If Medical Financial Services can not provide me with proof such as a tracking number that they attempted to lawfully validate this alleged debt with me, the consumer, prior to furnishing it on my consumer report, this has been a fraudulently furnished account with forethought and malice as an attempt to bring harm to me and my financial reputation. I am legally refusing to pay this debt pursuant to 1692c ( c ). I am demanding a cease and desist of all illegal activity, any communication and collection activity, of this and any alleged debts until Medical Financial Services Inc can provide me with sufficient documentary evidence that I have any legal obligation to pay them. If this documentary evidence can not be produced and Medical Financial Services Inc continues its collection efforts, I will file for litigation for actual damages caused and Medical Financial Services will be held criminally liable for aggravated identity theft pursuant to 18 USC 1028A, extortion, theft by deception, securities fraud, and mail fraud. Upon the receipt of the documents herein and of this notice, you are hereby notified Pursuant UCC 1-202f. Your prompt attention and response are both requested and required.

Complaint What Happened My name is XXXX XXXX. I tried to choose the closes issues that you listed to mine. They do n't exactly fit my situation but I hope I 've explained my issue enough so that you understand it. I was taken to XXXX XXXX Hospital on XX/XX/XXXX. I was taken to XXXX XXXX by ambulance, which was called by the Medical facility in which I had a dr 's appointment for that day. I was taken to XXXX XXXX at XXXX XXXX XXXX XXXX MS XXXX. I received a bill from XXXX XXXX Hospital a few weeks later which was about {$4000.00}. I thought that was very high so I called my insurance company and they told me that it was out of network and that 's why they did n't pay but a small portion of the bill. I told the Insurance rep that it was not my fault that I was taken to this hospital by ambulance and that it should be treated like an " in network hospital '' and they agreed. They insurance rep told me that they would be resubmitted the claim as " in-network ''. They very same day, I called XXXX Hospital and told them that they insurance rep told me that he was submitted the claim again. The XXXX XXXX hospital rep said she was typing this information in the notes and that if they needed more information they would contact me. I told the rep that if I owed anything further to please send me a new bill and she stated they would send a new bill if I owed anything further. I never received ANY other bill that there was still a balance owed to XXXX. The next correspondence I received regarding XXXX XXXX Hospital was from Medical Financial Services , Inc.I did n't believe this to be true because XXXX never sent me any further correspondence. After getting several letters from Medical Financial Services, I decided to ask for verification of this account. They send me the attached papers. I was thinking that there 's no way I owe them because XXXX would have sent me another statement if I stilled owed them once the Insurance company re-filed my claim. XXXX should have a record of this call and notes as the rep stated she was taking notes and if there was anything I owed, they would mailed me a letter which I never received after the 2nd claim. I disputed this with Medical Financial and the credit bureaus. My 2nd issue is that I never gave XXXX XXXX permission to give Medical Financial Services , Inc any of my medical information. I never signed any paperwork for them to do so or gave permission for anyone to sign. I have written XXXX XXXX 3 certified letters with my complaints and they have not responded to one of them. Medical Financial sent me a letter saying they would not respond to me any further because they have already sent me validation. No the papers they send me IS NOT validation that I owe them any monies. If I owe XXXX XXXX Hospital, they should have sent me a statement of what I owe them after my insurance company did another claim for my visit. I did not sign a contract with Medical Financial nor do should they have ANY of my medical information. If I owe XXXX, I should have been allowed to pay THEM in which I had services rendered. I feel that my medical privacy was violated in this matter. This could have been avoided if XXXX XXXX would have just let me known that there was still a balance instead of sending it strait to collections and Medical financial services placing it on my credit. If I indeed owe XXXX I have no problem with paying XXXX. This should not have went to collections and I was properly notified of another balance and I did not give them permission to share my records.
