One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. (a) Press Department, Veneer Glue Spread Layup Operation, North End of Discharge Roller Conveyor - On or about April 22, 2015 employees were exposed to getting caught in/by the first two live, powered rolls which were about 4 feet wide and 3 feet above the floor due to the employee's inability to access an emergency stop cord. |

29 CFR 1910.304(g)(6)(vi)(C)(5): Exposed noncurrent-carrying metal parts of cord- and plug-connected equipment that could have become energized were not grounded under the following condition(s) listed in (C) of this paragraph, i.e., (5) cord- and plug-connected appliances used in damp or wet locations, or by employees standing on the ground or on metal floors or working inside of metal tanks or boilers: (a) Press Department, Veneer Glue Spread Layup Operation, Northwest Corner of Infeed Core-Stack Scissor Table - On or about April 22, 2015 employees were exposed to an ungrounded extension cord supplying power to one of two overhead pedestal fans via a yellow AWG 12/3 extension cord having a 3-way female end but which was missing the grounding prong at its male attachment plug. |

(a) Press Department, Veneer Glue Spread Layup Operation, Northwest Corner of Infeed Core-Stack Scissor Table - On or about April 22, 2015, employees were using extension cords as a substitute for fixed wiring due to the lack of a sufficient number of hard-wired outlets in the area. Extension cords were powering two overhead pedestal fans via an orange AWG 12/3 extension cord and a yellow AWG 12/3 extension cord. |
