(a) Throughout Mill Room - On or about July 7-8, 2015, employees working throughout the Mill Room were exposed to fire hazards due to extension cords being used as substitute(s) for fixed wiring, to wit: (1) West wall, North end - the East-most 30 inch diameter pedestal fan at the Ultimizer's Feed Chain was connected to a 30-40 foot long homemade AWG 12-3, Type SO extension cord, (2) West side of Mill Room at Chop Saw - The pedestal fan's cord was wrapped around fan's housing and then was connected to a ~15 foot flexible cord which ran up to another flexible cord which ran over and down to an elevated quad-receptacle located above the infeed conveyor rollers, and (3) on West side of the Shoda Twin-head Router, a Fasco 30 inch pedestal fan was connected to a duplex outlet on South wall via a ~25 foot long AWG 16-3 Type SO cord, which was being used as a substitute for fixed wiring.

Citation text line 29 CFR 1910.307(c)(2)(i): Equipment in hazardous (classified) location(s) was not approved for the ignitable or combustible properties of the specific gas, vapor, dust, or fiber that was or could be present: (a) Throughout Mill Room - On or about July 7-8, 2015, employees working throughout the Mill Room were exposed to fire and explosion hazards due to electrical equipment and wiring in the following areas not being dust-tight, ignition-proof (i.e., dust ignitionproof) and approved for Class II, Division 1 & 2, Group G Combustible dust locations, in that general purpose wiring components and appliances were used throughout, e.g.: (1) numerous general purpose NEMA-1 duplex and quad-outlets, not approved for Class II hazardous locations, were powering 30 inch diameter pedestal and wall-mounted fans; and (2) numerous brands of 30 inch diameter pedestal and wall-mounted fans (120 VAC, 60 HZ, 200 Watts), not approved for Class II hazardous locations, were being used for air-movement and personnel cooling.
