29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, shall be kept cleared from work areas, passageways, and stairs, in and around buildings or other structures. (a) Construction Site - On or about December 21, 2012 employees were exposed to multiple fall and puncture hazards while working. Employees were exposed to falling on wood boards and wood boards with nails protruding from them. There were seven instances of wood boards with protruding nails observed in different locations. |

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as follows: (a) Front of House - On or about December 21, 2012 one employee was exposed to a fall hazard while working a wooden fabricated scaffold with a walkway that was not fully decked or planked. There was a 26 inch opening between two wood boards used scaffold work deck boards. |

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers. Note to paragraph (g)(1): The fall protection requirements for employees installing suspension scaffold support systems on floors, roofs, and other elevated surfaces are set forth in subpart M of this part. (a) Front of House - On or about December 21, 2012 two employees were exposed to a fall hazard while working on a wooden fabricated scaffold 11 feet 6 inches above concrete floor. Employees were not protected form falling to lower level by a guradrail system or personal fall arrest system or safety net system. |
