A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that are 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees. (a) Jobsite - On or about February 11, 2015 employees worked the entire length of a trench 63 feet long x 5 feet 06 inch to 7 feet deep x 7 feet wide with vertical sides. The only egress was the 45 degree cut ramp on the east end.

Employees working in type B soil were not protected from cave-in hazard by a trench shield protective system or sloping and benching system along the entire length of the trench.
