(a) In back of the building - On or about August 3, 2017 two employees were exposed to a fall hazard due to a wooden board and siding being placed on the scaffold platform to increase the working height. The scaffolds platform height was 14 feet 1 inch high. The total fall height was 16 feet 1 inch above the ground.

Several boards were not installed on the scaffold resulting in an 18 inch gap where the employees could fall 13 feet 6 inches on to concrete.

Citation text line 29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access. (a) Canopy - On or about October 21, 2014 two employees were exposed to a fall hazard due to a scaffold not having a ladder installed. This scaffold was measured at 13 feet 6 inches above concrete. Employees were observed climbing on the frame and standing and cross-braces. The Upton Plastering Company was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.451(e)(1), which was contained in OSHA inspection number 315308148, citation number 1, item number 1 and was affirmed as a final order on May 22, 2012, with respect to a workplace located at 337 East First Street, Forest, MS 39074.
