(a) Dry Kiln - On or about July 23, 2014 the employer did not maintain a Hyster 155 XL2 Forklift serial no. F006V01614Y in a safe condition. This forklift was operated with 1) two worn rear tires, 2) a brake system with missing parts 3) a cracked radiator water bottle 4) fire extinguisher not fully charged. |
