When the periphery of the blades of a fan is less than seven (7) feet above the floor or working level, the fan's blades were not guarded by a guard which had openings no larger than one-half (1/2) inch. (a) Finishing Area, west of Welding Enclosure - On or about June 30, 2016 employees were exposed to injury from fan blades due to a 5 11/16 inch diameter opening in the fan blade guard's center caused by the absence of, or due to, the missing emblem cover on the 30 inch diameter TPI Pedestal Fan, Model NSC 243, S/N 450408. |
