29 CFR 1917.43(c)(5): Powered industrial trucks were not maintained in safe working order, in that: safety devices were removed or made inoperative. (a) Port of Gulfport, West Pier, Chiquita Warehouse, Banana Transfer Area, Shed 16 - On or about March 7, 2014, employees operating Hyster Powered Industrial Truck #FL7016 were exposed to injury due to the absence of an operational horn. The horn cover at center of steering wheel was missing, and no mechanism was present to make the horn sound. (b) Port of Gulfport, West Pier, Crowley Chassis Maintenance & Repair Shop - On or about March 7, 2014, employees operating Hyster Powered Industrial Truck #FL9060 were exposed to injury due to the seatbelt not being functional. The belt would not retract or tighten around the operator's waist. |

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure: (a) Port of Gulfport, East Pier, Dole Maintenance & Repair Shop - On or about March 6, 2014, employees were exposed to injury due to the use of the following extension cord as a substitute for the fixed wiring due to an insufficient number of duplex outlets: (1) a 100 foot round orange AWG 12/3 Type SJTW extension cord, with a three-way female plug, was supplying current to the Dayton 120,000 BTU Portable Oil Fired Heater, the Seven (7) inch Metal-cutting Bandsaw, and the Magic Chef Microwave Oven; and (b) Port of Gulfport, West Pier, Crawley Chassis Maintenance & Repair Shop - On or about March 6, 2014, employees were exposed to injury due to the use of the following extension cords as substitutes for fixed wiring, due to an insufficient number of duplex outlets to power a refrigerator, microwave oven, fan, Sawzall, and portable hand-held right-angle grinder: (1) a 50 foot round orange AWG 12/3 extension cord, (2) a 100 foot round orange AWG 16/3 extension cord by Workforce, and (3) a black AWG 12/3 extension cord with a quad-receptacle box at the female end. |
