(a) McFarland Timber Tract #C-030 Clarke County, Mississippi - On or about February 4, 2013, employee(s) bleeding air from the hydraulic transmission line at the hydraulic fluid filter of a 1998 Franklin Skidder, Model 170 PTM-31A-DLX, with the engine running, were exposed to being fatally crushed by the sudden achievement of prime and the skidder's forward movement because energy control procedures had not been developed and implemented to achieve full employee protection, i.e., preventing the machine from moving while the employee was working on it from the ground.

Citation text line 29 CFR 1910.253(b)(2)(ii): Inside of buildings, cylinders shall be stored in a well-protected, well-ventilated, dry location, at least 20 (6.1 m) feet from highly combustible materials such as oil or excelsior. Cylinders should be stored in definitely assigned places away from elevators, stairs, or gangways. Assigned storage spaces shall be located where cylinders will not be knocked over or damaged by passing or falling objects, or subject to tampering by unauthorized persons. Cylinders shall not be kept in unventilated enclosures such as lockers and cupboards. (a) Mechanic's Shop at office complex, 47 County Road 261, Shubuta, MS 39360 - On or about February 12, 2013, employees were exposed to a struck by hazard from two unsecured 150 cubic feet acetylene cylinders, one unsecured 307 cubic feet acetylene cylinder and two 56 inch high oxygen cylinders.
