| Citation text line | 29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury (a) North driveway of New Kroger Building - On or about November 4, 2015 employees installing steel beams used a forklift to placed a 64 foot beam partially on a asphalt driveway and unstable dirt at a 2 degree slope. The unsupported beam fell over onto the leg of an employee placing blocks a few feet east of the beam. The beam amputated the employee left leg below the knee. |
