This was not accomplished by natural or mechanical ventilation with discharge or exhaust to a safe location outside of the building. Provision was not made for introduction of makeup air in such a manner as not to short circuit the ventilation. Ventilation was not arranged to include all floor areas or pits where flammable vapors may collect. (a) Lamination Mixing Room - On or about January 27, 2016 the mixing room did not have mechanical ventilation or adequate natural ventilation. |

(i) R & D Spray Area - On or about January 27, 2016 electrical receptacles, electrical panels on the outside of the paint booth, bench grinder and battery chargers on work table within 20 feet of the spray area were not explosion proof. (ii) Lamination Small Parts - On or about January 27, 2016 electrical panels, electrical disconnects and exhaust fan on the wall between the ventilation systems were within 20 feet of the spray area and were not explosion proof. (ii) Lamination Molding Paint Booths - On or about January 27, 2016 fluorescent light fixtures and wiring were not explosion proof. Gel coat residue was on the light fixtures and on the wall at or above the light fixtures. (iv) Mixing Room - On or about January 27, 2016 fluorescent light fixture, orange extension cord and radio was not explosion proof. |
