29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. (a) East interior side of building - On or about December 9, 2016 employees working from scaffolding 19 feet high gained access without the use of a ladder by climbing up the side braces of the scaffolding. The same employee was exposed in 2012 was exposed in 2016. The S&S Masonry, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(e)(1), which was contained in OSHA inspection number 392343, citation number 1, item number 1and was affirmed as a final order on April, 4 2013, with respect to a workplace located at 200 W. Main St. Broussard, LA. 70518. |

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: (a) Inside east wall - On or about December 9, 2016 employees building a block wall were allowed to work from welded tubular framed scaffolding platform that was 19 feet high with no fall protection or guard rails on all open sides.
