(a) Rear of Building, Outside - On or about September 4, 2013 employees were exposed to crushing hazard from tip-over while operating a forklift transporting loads. Clarke forklift (Model C500Y60, serial Number Y685-467-2153) was not equipped with a seat belt in the operator's seat. |
