(a) 5th floor window ledge - On or about November 28, 2017 two employees were exposed to a fall hazard while standing in an unauthorized open wooden box used as a scaffold. This box was elevated 41 feet 5 7/8 inches above concrete by a Caterpillar TL 1055C Telescopic Forklift serial no. MR1820. This box was not designed by the manufacture to support employees.

Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews. (a) 5th floor window ledge - On or about November 28, 2017 two employees were exposed to a fall hazard while working in an unauthorized open wooden box used as a scaffold. This box was elevated by a Caterpillar TL 1055C Telescopic Forklift serial no. MR1820. These employees were elevated 41 feet 5 7/8 inches above concrete with no fall protection. The front end of the box was opened.
