Rodent droppings were present on top of and on floor around air vent for HVAC system. Filter grills for HVAC system were caked with dust and grime. Visible mold was on present on wall outside of restroom near baseboard. Citation text line 29 CFR 1910.141(a)(5): Enclosed workplace(s) were not so constructed, equipped, and maintained to prevent the entrance or harborage of rodents, insects, or other vermin: (a) Office - On or about October 18, 2018 employees were exposed to vermin and there droppings. Droppings were on top of and on floor around air vent for HVAC system that controlled climate in the office. There is an opening at the bottom left corner of the roll-up door and the bottom of the rear exit door did not reach the floor leaving openings for rodents, insects and other vermin to enter at will.
