29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, shall be kept cleared from work areas, passageways, and stairs, in and around buildings or other structures. (a) Construction Site - On or about December 19, 2012 employees were exposed to multiple fall and puncture hazards while working. Employees were exposed to falling on boards and boards with nail protruding from them. Six instances with boards with nails where obsered in different locations. |

29 CFR 1926.451(g)(1)(iii): Each employee on a crawling board (chicken ladder) shall be protected by a personal fall arrest system, a guardrail system (with minimum 200 pound toprail capacity), or by a three-fourth inch (1.9 cm) diameter grabline or equivalent handhold securely fastened beside each crawling board; (a) Front Right Scaffold - On or about December 19, 2012 employees were exposed to a fall hazard of 13 feet while working on a crawling board (chicken ladder) scaffold. Employees were not protected from falling to lower level by a personal fall arrest system or a guardrail system. |
