(a) Construction Site - On or about December 19, 2012 employees were exposed to multiple fall and puncture hazards while working. Employees were exposed to falling on boards and boards with nail protruding from them. Six instances with boards with nails where obsered in different locations.

Employees were not protected from falling to lower level by a personal fall arrest system or a guardrail system.
