29 CFR 1910.107(c)(6): Electrical wiring and equipment not subject to deposits of combustible residues but located in a spraying area as herein defined were not explosion-proof type approved for Class I, group D locations and did not otherwise conform to the provisions of subpart S of this part, for Class I, Division 1, Hazardous Locations: (a) Building C - Paint Shop, Northeast corner of Bay #2 of 5 - on or about May 8, 2014, employees were exposed to electrical-source fires due to the presence of a NEMA 1 type duplex electrical outlet & light switch and a 100 foot round yellow AWG 12 extension cord inside the Control Room, whose open-door would not close; in addition there was a radio inside Bay #2 spraying area where small parts were being spray painted as they hung on fixed racks throughout the bay. (b) Production Building D - Paint Mixing Room - on or about May 8, 2014, employees were exposed to electrical-source fires due to the presence of the following non-explosion proof equipment in a small cubicle where multiple gallons of flammable tints, reducers and paints were being mixed: (1) a NEMA 1 electrical outlet box, with two duplex ground-fault receptacles; (2) a portable (clip-on) 120 VAC aluminum-hooded light-bulb fixture; (3) a Square D 600 Volt, 60 Amp, side-throw, Heavy Duty Safety Switch #PPI-14; and (4) a Fillon Technologies Modular Paint Mixing Machine. |

29 CFR 1910.252(b)(1)(i): A welder or helper working on platforms, scaffolds, or runways was not protected against falling: (a) Production-Fabrication building, Sub-Arc Area, Elevated Sub-Arc Automatic Welder Operation - On or about May 9, 2014, the welder-employee sitting on elevated work-platform with 18 inch high guard-rails was not adequately or effectively protected from falling 8 feet to floor below. The employee was provided a personal fall-arrest system (PFAS); however, the PFAS lanyard's anchor-point was slightly above and in front of the employee instead of above and behind, the six-foot lanyard with shock-absorbing rip-stitching would not provide protection from an 8 foot fall and the carrier-beam for the automatic seam welder to which the lanyard was anchored was not structurally anchored sufficiently to withstand all forces acting against it, should the employee fall, or the hydraulic hoses or cylinders fail. |
